Instructions
In the process of assessing for treatment response, we often repeat questionnaires to look at your symptoms
following changes in medications or circumstances. These can be compared against the forms done during your
initial assessment to see how your symptoms have changed.

There are usually only 3 forms (Adult ADHD Self-Report Scale, Copeland Symptom List, DASS-21), but
sometimes you may be given additional forms depending on the situation.

For all of these forms, please complete them considering your symptoms while your medications are active.
For example, if you struggle with concentration, but the medications you take improve your concentration, rate
the scores based on the medication effect at best. Sometimes the medications work well, but the duration is
short lived – if so please discuss with your doctor.
